If an individual has any concerns about patient care and safety in the hospital
(that have not been addressed), he or she is encouraged to contact the hospital’s
management at 919-690-3405 (administrative assistant) or 919-690-3418 (risk manager).

If the concerns cannot be resolved through the hospital, you may contact the Joint Commission’s Office of Quality Monitoring to report any concerns or register complaints by either calling 1.800.994.6610 or e-mailing: complaint@jcaho.org.
